About 4.40pm, an 88-year-old man was crossing Bass Street in a southerly direction when he collided with a Hyundai sedan travelling west.
The man struck the bonnet of the car, before being thrown onto the ground.
The 49-year-old female driver stopped at the scene and contacted emergency services.
The injured man, from Shellharbour, was treated by paramedics before being taken to Wollongong Hospital with head and spinal injuries.
The female driver from Barrack Heights was taken to Shellharbour Hospital for mandatory blood and urine tests.
Officers attached to Lake Illawarra Local Area Command attended and established a crime scene.
