The crash occurred shortly before 2pm, about four kilometres south of the Macarthur Drive intersection, when a Mitsubishi Colt and a silver Mazda 626 travelling in opposite directions collided.
The 66-year-old woman and 68-year-old man were in the Mitsubishi and died at the scene.
The 18-year-old driver and only occupant of the Mazda was airlifted to Liverpool Hospital with serious injuries.
Picton Road was closed in both directions between Macarthur Drive and the M1 for several hours while emergency services worked to clear the area.
The accident was the first fatality along the stretch of road since works to improve safety were finished last year.
The Crash Unit Investigation team are looking into the cause of the incident.
