Monday's fatal crash is the second time a car has ploughed through a bus stop outside the Kogarah chemist on Railway Parade.

In March 2007, fashion student Emma Hansen was walking under the dark-green awning of O'Reilly and Daly Chemist when a red Toyota Echo mounted the kerb and crashed into a bus queue. The 20-year-old from Orange, on a lunch break with her friend, was killed.

Eleven others, including a toddler, were injured. An elderly man had a leg amputated.

Following investigations into the crash, Kogarah City Council, Roads and Maritime Services (formerly RTA) and NSW Police agreed there "no feasible engineering solutions would have prevented that crash occurring".

A woman has been charged following the yesterday's fatal crash at Kogarah, according to a police statement.

The driver, a 26-year-old woman was arrested and underwent mandatory blood and urine testing at hospital before she was taken to Kogarah Police Station.

She was interviewed by officers from the Metropolitan Crash Investigation Unit before being charged with a number of driving offences including dangerous driving occasioning death, negligent driving occasioning death and never-licensed person driving vehicle on road, police said.

The woman was given bail and will appear in Kogarah Local Court on 14 October.

The 29-year-old female passenger, and owner of the vehicle, was treated in hospital for shock.

Two young children – aged two and four – who were also in the car were not injured.

Meanwhile, a critical incident investigation team has been established to investigate all circumstances surrounding the crash.

In May 2007, the speed limit along Railway Parade was reduced from 60km/h to 50km/h.

The 2008 inquest into Ms Hansen's death heard that a 42-year-old learner driver was at the wheel. Rose Deng panicked when her neighbour John Titmarsh – a licensed driver, but not a driving instructor – placed his hands on the wheel as she took the corner too widely.

She hit the accelerator instead of the brake, crashing into a line of people standing on the opposite side of the road to Kogarah train station.

The mother of five avoided criminal charges and was instead dealt with under the Mental Health Act.

In December 2008, the coroner recommended that a list of 12 tips for better learner supervision be added to learner driver log books.

He did not make any recommendations on the layout of the pedestrian and traffic bend.

Kogarah Chamber of Commerce president Megan Barker said Monday's accident was "a repeat of seven years ago".

"For one, we need signage so that cars coming around Railway Parade north know they have to give way to traffic coming down Montgomery Street, that they have to put on their right blinker. There's no signage, nobody knows that," she said.

She also wants the bus stop removed. "It's right on that bend and it's a blind spot. It was an accident waiting to happen."
