Shields beat Dapto-Dandaloo's grand final-winning midfielder Sam Munro to win his second consecutive medal.
The attacker used a league-best 27 goals to claim the crown.
He won the title in his first season in the Illawarra Premier League while playing with United.
Runner-up Sam Munro had to be content with the bridesmaid tag for the third time after finishing second in 2011 and 2012.
In other awards, Hayden Durose (Wollongong Olympic) took out the Golden Glove Award, while Bulli gaffer Ben Smith won Coach of the Year.
Bulli were also awarded the Club Champions and Fair Play awards.
Jack Lombo (South Coast United) was youth grade Player of the Year, while Dylan Lewis and Brendan Pattman Bulli shared the Golden Boot with 18 goals.
In the District League, Jason Rea of Corrimal Rangers won the Neville Arrowsmith Medal.
Shell Cove boss Robert Mihailov took out Coach of the Year, Thirroul's Jayden Lewis won Golden Glove and Thirroul were named club champions.
Player of the Year in youth grade was shared between Zac Nicholas (Wests Illawarra) and Kalen Pupovac (Corrimal Rangers). Balgownie Rangers claimed the Fair Play Award.
Top scorers were Shell Cove's Ile Kostovski (17 goals) first grade, Balgownie Ranger Joshua Faulks (34 goals) reserve grade and Corrimal Ranger Timothy Wylie (25 goals) youth grade.
