Emergency services were called to a collision on Appin Road near West Cliff Colliery about 6.20pm, following reports a ute and car carrier had crashed.
The male driver of the ute, believed to be a 27-year-old Sydney man, died at the scene. The male truck driver was assessed on site by ambulance paramedics before being taken to hospital for tests.
A photograph of the wreckage posted on social media shows a ute with a crushed cabin and debris strewn around the site.
Chief Inspector Shane Howard, of Campbelltown Police, said investigating officers were not in the position to release any more information about the crash or the 27-year-old man on Sunday evening.
"We're still doing the identification process," he said.
"It was quite horrific [on Saturday] and we've still got some investigation to do on both sides."
Several people have died on Appin Road in recent years, including a woman who was killed in a two-car crash at Rosemeadow, just south of Campbelltown, on September 17.
Illawarra Stingrays player Ashleigh Connor died after her car veered to the right and crashed into a tree on the opposite side of the road in July, 2011.
In June, 2014, Roads and Maritime Services started work to install a 360-metre guard rail along a notoriously deadly stretch of the road.
