Angela Lee Hood, 30, was at the courthouse on unrelated matters when she was arrested outside the building shortly after 10am.
She is accused of being behind the wheel of a Renault hatchback which struck a stationary four-wheel-drive parked on the side of Gerroa Road on March 29.
Police and paramedics were called to the roadway just after 8am to find one car on its side.
A 40-year-old male passenger in the Renault suffered critical injuries in the crash and later died.
On Wednesday, Hood was charged with causing bodily harm by misconduct while in charge of a motor vehicle, aggravated dangerous driving, aggravated dangerous driving occasioning death while under the influence of drugs, and two counts of drug possession.
The 30-year-old did not apply for bail when she appeared in court via video-link late in the afternoon.
Defence lawyer Matt Russoniello said his client, who was walking with the help of crutches, was in a great deal of pain from a dislocated hip.
"She's not in a particularly wonderful state, as one can imagine," he said.
Magistrate Michael Stoddart made an order for her to receive medical attention while in custody.
The court also heard Hood had a close connection to the deceased person. "The person that was killed in this accident was a very close friend of hers," Mr Russoniello said.
The matter was adjourned for mention on Friday.
