About midday on Saturday, a man and a woman, both 21, were walking along Frampton Avenue, Marrickville, when they started arguing.
A 55-year-old man intervened and spoke to the 21-year-old man who allegedly shouted at the older man, before hitting him in the face with a scooter.
The older man fell to the ground, while the attacker walked on, before throwing the scooter into the front yard of a nearby house, police said.
The man was taken to Royal Prince Alfred Hospital with head injuries. He remains there in a critical but stable condition, police said.
Police arrested the 21-year-old man and 21-year-old woman on Illawarra Road, Marrickville, but they were later released pending further inquiries.
On Sunday morning, officers again attended a property on Illawarra Road, Marrickville, and re-arrested the man.
He was taken to Marrickville police station and charged with recklessly inflicting grievous bodily harm. He was refused bail to appear before Newtown Local Court on Monday.
