Emergency services responded to the accident and police from Camden Local Area Command found the driver of one car, a woman, was deceased.
The passenger of the same car, also a woman, has been taken to hospital in a serious but stable condition.
The driver of the other car, a man, has been taken to hospital in a critical condition.
Early investigations indicate that the two cars collided head-on.
Police are appealing for anyone with information about the incident that may assist investigators to call Crime Stoppers.
A crime scene has been established and will be examined by Metropolitan Crash Investigators.
Picton Road is currently closed in both directions, between Macarthur Drive and the Princes Motorway. Motorists are urged to avoid the area and check www.livetraffic.com for updates.
Meanwhile, a 17-year-old has been airlifted to hospital after she was hit by a ute and trapped beneath it in the Southern Highlands.
The girl was freed by emergency crews when they arrived to an education facility at Moss Vale just before 6pm, and she has been taken to Liverpool Hospital in a serious but stable condition, police say.
It’s believed the girl and the driver, a 55-year-old man, were working on the ute when the driver moved it forward, hitting the girl.
