NSW Ambulance paramedics were called to a crash on Central Avenue, near the Fisher Street roundabout, about 9.55am.
They found the woman, 78, with a head injury and multiple injuries to her lower body.
She was treated at the scene and transported by road ambulance to Illawarra Regional Airport, then airlifted to St George Hospital, where she remained on Thursday evening in a critical condition.
The crash occurred after the woman stopped off a designated walkway in the middle of the road.
She collided with a southbound Holden Commodore SS, with a 23-year-old woman at the wheel.
A police investigation into the matter continues.
The driver was taken to Shellharbour Hospital for mandatory tests.
Officers from the Metropolitan Crash Investigation Unit are investigating the circumstances surrounding the crash.
