Nine ambulances and three helicopters are attending the scene after being called around 12:30pm on Tuesday, with reports of a collision between four cars and two trucks.
NSW Ambulance Superintendent Kathy Golledge said it is "absolute carnage".
"One car is believed to be completely crushed under a truck," she said.
"We are treating multiple patients, some are still trapped by compression at this stage.
"The road is closed and an exclusion zone in place while we work to treat the injured."
A NSW Police spokesperson told Fairfax Media 12 people in total are involved in the crash.
In one car was a man, 20, a woman, 24, and a two-year-old child. All were listed as being stable and have been taken to Dubbo Base Hospital.
A 26-year-old person in another car had suffered pelvic injuries and was also taken for treatment. A 52-year-old suffered multiple injuries.
A truck driver has been flown to hospital with a fractured leg and other injuries. The two people trapped are alive and conscious.
NSW Police have also clarified that no roadworks were involved in the crash, despite NSW Roads Minister Melinda Pavey earlier telling ABC News that was the case.
The Newell Highway is currently closed in both directions and NSW Police are warning motorists to avoid the area.
The crash is the third fatal incident involving trucks on NSW roads in the past 24 hours. On Monday, one man was killed in a fiery collision involving three trucks on the M1 Pacific Motorway near Cooranbong, which sparked a bushfire. Another two truck drivers died later that afternoon in a head-on collision near Grafton.
Police are urging anyone with information on the Dubbo incident to call Crime Stoppers on 1800 333 000.
