The man, who cannot be named for legal reasons, is alleged to have subjected the woman to vile and degrading physical and verbal abuse during the two-day ordeal and repeatedly threatened to kill her and their five-year-old son.
At one stage, he allegedly asked the woman how she wanted to die - by bashing, stabbing or being set on fire.
"I have been planning how I will kill you, I would hang you and write a letter saying you can't put up with it anymore," he allegedly said during the ordeal.
Court documents said Silverwater jail staff contacted the victim on October 9 and asked her to pick up her ex-partner, despite the fact there was a no-contact AVO between them.
The woman collected him and they spent the rest of the day together, with their young son, without incident.
However, it is alleged two days later the man discovered his ex was dating someone else and flew into a rage, calling her a slut and threatening her with a knife.
He apologised after a brief but intense barrage of abuse and the following day was incident-free, police said.
But it is alleged the man became enraged over the situation again and turned up at the woman's house when she arrived home from work in the early hours of October 13.
He allegedly forced her out of the driver's seat and got inside, before driving her around in the vehicle, allegedly hitting and threatening her multiple times.
"I'm gonna sell you to my friends for ice," he allegedly told her.
"I'm going to kill you....I'm recording it, I'm going to post it on Facebook so everyone can see."
It is alleged he forced the woman to perform fellatio on him during the drive while calling her degrading and derogatory names.
The victim told police the ordeal continued for the rest of the night, as her ex drove her to multiple places including his brother's house, a carpark and a service station.
The pair eventually ended up at the Wollongong methadone clinic, where the man again allegedly threatened to kill the woman if she tried to run away.
However, she told police she managed to escape while he was inside the building getting his methadone shot.
She ran to Wollongong Hospital and alerted staff, who phoned police. The man was arrested on Tuesday at a house in Mangerton.
He was charged with 25 offences including sexual intercourse without consent, kidnapping and domestic assault.
The man applied for bail in court on Wednesday, saying he strongly denied the allegations against him and needed his liberty to attend a relative's funeral and celebrate his son's upcoming birthday.
"He's willing to abide by any conditions imposed by the court - he'll report three times a day and is willing to wear an ankle bracelet," defence lawyer Anthony Stewart told the court.
"I'm asking your Honour to give him a chance."
However, Magistrate Susan McGowan noted the case against the man appeared a strong one, saying her injuries seemed consistent with her account of what had happened.
She refused bail on account of the man's lengthy criminal record and the likelihood of a custodial sentence if he's convicted.
The case will return to court on November 13.
