Your new role
As a Chronic Disease Management Health & Wellbeing Nurse your main duties and responsibilities would include preparing GP Management Plans with patients on behalf of the GP, preparing Team Care Arrangements often alongside the GPMP and conducting health assessments to assist in early identification of chronic disease. You role will also include:

  • Providing nurse monitoring and support to eligible patients, often as part of the GPMP, TCA or Health Assessment.
  • Identifying additional appropriate services to patients and perform as required.
  • Coordinating care across the practice, together with GPs, specialists and allied health professionals to provide a holistic approach to care.
  • Ensuring that all care provided to patients is of the highest quality and meets clinical standards as set.

What you'll need to succeed
You must hold an AHPRA registration as an Enrolled or Registered Nurse. A successful candidate would also hold the following attributes:

  • Proven prior experience in Chronic Disease Management and health assessments.
  • Knowledge of Health Assessment is highly regarded.
  • Strong organisational and time management skills.
  • Outstanding communication and customer service skills.
  • A passion for people and a personable approach.
