The pursuit began when a silver Nissan Pulsar took off from a routine police stop about 3.50am on Richmond Street at Merrylands.
Police followed the sedan through Smithfield, Liverpool and Campbelltown, before calling off the chase at Broughton Pass.
But about 40 minutes later the car was seen coming down Mount Ousley Road and the chase resumed.
Wollongong Inspector Phil O'Neil said the sedan continued down the mountain before turning into Ajax Avenue in North Wollongong, where it turned around and went back up Mount Ousley.
Two Wollongong police cars followed the sedan along Appin Road.
The pursuit came to an end about 5am in Woolmers Place at Glen Alpine, when police cornered the vehicle.
"Police tried road spikes to stop the vehicle, but the offending driver took some evasive actions and got past that," Insp O'Neil said.
"He then went to a dead-end street and police vehicles tried to block the path for when they came out."
Three men were arrested at the scene, while a woman was tracked by a police dog unit and arrested in a nearby street.
Police identified the sedan as stolen and allege the driver rammed the car into a police vehicle.
All four were taken to Campbelltown police station. Some charges, including take and drive conveyance, have been laid.
Insp O'Neil said a senior constable was taken to Campbelltown Hospital to be checked for possible arm, knee and back injuries, but has since been released.
