The woman was the sole occupant of a dark blue Mazda 121 that was heading south down Mount Ousley Road about midday Monday.
Reports from the scene indicate the car ran into the back of a truck that was carrying two men.
The truck's passenger, Adam, told the Mercury he felt the impact as the car made contact from behind.
"We were coming down the hill and she's come up the back of us. It was a big hit," he said.
"Her car was wedged under the truck.
"She looked in a pretty bad way."
The Mercury understands conjoined car and truck continued down the hill for about 250 metres before the truck came to a halt and the car broke away.
An ambulance rescue chopper has landed at UOW to collect the 66-year-old female Mazda driver, who suffered facial, chest and head injuries in the crash.
A doctor from on board the chopper is treating the woman on UOW's sports oval.
The truck's occupants were not injured.
The medium rigid truck, which was carrying freight at time of impact, has been towed from the site with minimal damage.
The badly damaged Mazda has also been towed and the roadway is clear.
UPDATE, 6pm: The woman has been airlifted to St George Hospital, where she remains in a critical but stable condition.
CORRECTION: An earlier version of this story stated the injured woman was aged 55. St George Hospital has since confirmed the woman is 66.
