Emergency services have conflicting reports as to whether the man died after falling from a cliff, or was washed off a rock ledge.
A NSW Ambulance spokesman said six ambulance crews and a helicopter responded to the emergency after a caller indicated their friend had fallen off a cliff and into the water at Little Marley Beach at 3.40pm.
NSW Police said they were told two men had been washed off a rock ledge into the water but one was able to climb back up.
The other man, whose age is unknown, was located face-down in the water near the shoreline shortly after 4pm.
He was winched out by the helicopter, which landed nearby while a medical team worked to save him, but died at the scene.
Little Marley Beach is within a popular coastal walking track, with visitors regularly taking selfies at the cliff's edge.
The NSW Ambulance spokesman said it was the second incident involving an apparent cliff fall in the space of just 10 minutes.
A 34-year-old man was lucky to escape serious injury when he fell about 20 metres from a lookout at Jamberoo.
The call came through to NSW Ambulance at 3.34pm, just six minutes before the fall at Little Marley Beach.
Although initially knocked unconscious in the fall, the patient at Jamberoo was able to talk with emergency responders after they arrived.
