The 16-year-old from the Gymea area was heading north on Lawrence Hargrave Drive around 3.20pm, when police said he failed to negotiate a bend and collided head-on with a truck.
Wollongong Police District Inspector Don Faulds said the teen suffered life threatening injuries including major trauma to his head, fractures to his pelvis and multiple other fractures.
He was transported by helicopter to St George Hospital.
As at 6.30am Thursday the victim was in a serious but stable condition, according to a hospital spokeswoman.
Inspector Faulds said the truck driver had also been taken to Wollongong Hospital for mandatory testing.
Corrimal Traffic and Highway Patrol were continuing to investigate the matter.
The incident forced the closure of Lawrence Hargrave Drive in both directions between Otford Road and Chellow Dene Avenue for around two hours.
Anyone with information, including dash-cam footage, is urged to come forward to police.
