Reports to NSW Ambulance suggest the man was hit by a car, however this is unconfirmed.
Paramedics were called to Five Islands Road about 4.30pm Wednesday.
They found the man, believed aged in his 30s, unconscious but breathing, with a closed head injury.
A medical team traveled by road to care for him at the scene.
He has since been transported to the airport at Albion Park, and is expected to be flown to a Sydney hospital.
Police are on site. One of two westbound lanes is closed.
Update: The injured man has been transported to St George Hospital with a serious head injury.
