A male police officer and a woman driver were critically injured in a head-on crash.
An investigation has begun into which vehicle crossed to the wrong side of the road in the accident, which occurred on New Illawarra Road about 2pm.
A third car was also involved in the crash.
The police officer, who is in his 30s, was returning to Liverpool police station in his own car after a training exercise, 9 News reported.
The woman driver was reported to be in her 60s.
A spokesman for NSW Ambulance said it took "some time to get access" to the drivers and extricate them from the smashed vehicles.
Both were in a critical condition when taken to Liverpool Hospital.

Acting Inspector Melissa Thompson called it a "devastating accident" and said it was "extremely upsetting for the first responders" to turn up to an accident involving a colleague.
