A Lake Illawarra police operation was under way near the Berkeley island on Friday afternoon.
Lake Illawarra Police District duty officer Sergeant Matt Brophy said the area near the island had been cordoned off to "prevent the man from escaping following a pursuit."
Officers and a police helicopter were still at the scene at 2.30pm.
Sergeant Brophy said police officers pursued a car after the driver failed to stop when instructed to do so about 12.30pm.
He said a man was in custody.
Officers used a boat to retrieve the man.
*Earlier police reports indicated a woman had swum to the island but they later confirmed it was a man.
