Incumbent Liberal MP Lee Evans has a fight on his hands against Labor's Maryanne Stuart.
Premier Gladys Berejiklian on Friday, the day before the election, was at Heathcote train station talking to residents.
Last week the premier redirected resources in the must-win seat ahead of polling day.
Recorded messages from Ms Berejiklian were sent to voters' phones, where the Premier urged people to back Mr Evans.
"Only Lee Evans will ensure Sutherland Hospital is upgraded," Ms Berejiklian's message said.
Mr Evans said he was not concerned about the vote.
"If there is a swing of 7.6 per cent then I deserve to be thrown out," he said.
"I am very confident that I will get over the line. I have spent the past two weeks at pre polling booths where I have been doing well.
"I'm proud of what I and the Liberal party has achieved and what we will achieve in the next four years."
Labor candidate Maryanne Stuart, who lives at Woronora Heights, said Mr Evans and the Liberals "have neglected our community".
"They have failed to invest in our local infrastructure and services, and now they want to splurge $2.2 billion on Sydney Stadiums," she said.
"The Liberals have taken us for granted by slashing funding to TAFE, the rolling closures of fire stations, the cutting of rail services and blow outs in hospital waiting times.
"If you want the government to invest in schools and hospitals, stop the rampant overdevelopment and fix the train timetable, vote Labor.
"I will fight for the services we deserve and put our community first instead of splurging $2.2 billion on stadiums."
The last two state elections have seen unusually large swings in Heathcote.
The seat was once held by Labor with a comfortable margin but the vote moved emphatically to the Liberals in 2011 when Mr Evans won with a swing of almost 22 per cent.
The Liberals retained the seat in 2015 but there was a hefty 11.4 per cent swing back to Labor.
Mr Evans lists some of his achievements as $173 million Heathcote Road upgrade, digital signaling system on the T4 Illawarra Line and new carriages and extra seats on the South Coast Line as well funding the stage one of the F6 extension.
He also secured $1.5 million for a new police station in Helensburgh and funding to upgrade the Garie Beach to Otford track in the Royal National Park.
Mr Evans said Labor's plan to spend $2.2 billion on a train tunnel from Waterfall to Thirroul was a "joke" and could not understand why the party then announced more parking at Helensburgh train station when it would be bypassed by the tunnel.
Ms Stuart said her priorities were stopping "Liberals rampant overdevelopment", easing the cost of living by reducing electricity prices and fixing the "unfair train timetable introduced by the Liberals in 2015".
Keira MP Ryan Park had his say on the marginal seat too. He said Ms Stuart had "given her all" over a number of years.
"No one has worked the Heathcote electorate as hard as Maryanne Stuart," he said. "If the people of Heathcote give her the privilege of electing her as their member on Saturday evening, they will be in very good hands. She is a fighter.
"Lazy Lee finds it very difficult to do any work at all.
"The fact the Premier is spending every other day in seat of Heathcote shows just how much the Liberals are worried."
Mr Evans sits on a 7.6 per cent margin, making it the NSW Liberals' seventh most marginal seat.
The electorate straddles parts of the Sutherland Shire on Sydney's southern fringe and beachside neighbourhoods north of Wollongong as well as taking in the Royal National Park.
