Union safety representatives have issued the Port Kembla Coal Terminal with several formal safety notices in the wake of an accident where a truck driver lost his leg.

PKCT does have the option to dispute the notices.
Workplace health and safety representatives from the CFMEU - the union that represents the locked-out coal terminal workers - had visited the site to inspect the yard where the driver was injured.
Three "provisional improvement notices" were issued by the CFMEU representatives to PKCT.
They related to "lack of dedicated site supervision" at the time of the accident, an "inadequate emergency response system" and an inadequate road transport management "to ensure vehicles are properly managed and maintained on site".
"There has been a serious breakdown in safety systems for yesterday's accident to occur," CFMEU district secretary Lee Webb said.
"PKCT management has given us no assurance that safety systems for rail receival and loading have been audited following Thursday's accident."
PKCT can rectify any issues raised in such notices or opt to dispute them.
In the latter case, the dispute is heard by regulator WorkSafe NSW, who makes a decision on the issue.
