The man faces 62 offences after alleged irregularities were found by police, including altering the hours he worked and removing pages from his document.
The truckie, 28, was detected by police while driving a semi-trailer south on the Hume Highway near Ingleburn about 1.50am on February 17.
Macarthur Highway Patrol officers spoke to the man about an unrelated matter when they discovered the alleged breaches in the driver's work diary.
The man was recently issued a further court attendance notice, including for five counts of driving while suspended; exceeding the heavy vehicle speed limit by more than 10km/h; and five counts of making false or misleading work record entries.
The man will appear in Campbelltown local court on July 2.
