After a decade in the AFL Sydney competition, the pioneering women's club leave with a third premiership after beating Manly-Warringah 7.6 (48) to 6.5 (41) at Blacktown International Sports Park.
Nikki Kennedy was awarded best-on-ground, kicking two majors, while Kiara White, Phillipa Davis and Cate Stanton all finished with goals.
It was party time on Sunday for the Saints, adding the 2019 cups to their 2011 and 2017 successes.
"We're just so happy to have finished with another premiership," president Casey Moane said.
"We've loved playing in the Sydney competition and it's unfortunate we won't be there next year, but right now we can celebrate."
The Saints were out to prove their standing in the division one league when they booted four goals to one in the opening term.
But the lead was cut to four points by half-time.
Manly were held scoreless in the third, before kicking three goals to two in the last as the Saints held on.
The league restructure has been designed to align men's and women's clubs statewide, pushing the Saints out of the Sydney competition as a result.
Instead, the Saints will now join forces with Figtree next year and have two teams in the AFL South Coast league.
The club has played a role in developing West Coast player Maddy Collier, as well as AFL Sydney premier division star Rylee McGartland and Giants Academy player Stanton.
