Emergency services were called to Walang Avenue, Figtree, just after 4.30pm following reports a car had hit a tree.
According to Ambulance NSW officials, there were reports of a female passenger, aged in her 80s, being trapped in the vehicle and suffering multiple injuries.
The woman was airlifted to St George Hospital in a serious condition.
Ambulance NSW Chief Inspector Illawarra Terry Morrow said an elderly couple were driving downhill and collided with another car.
"They've made their way down the road uncontrollably, hit a tree head-on and both elderly people were seriously injured in that accident," he said.
"The driver of the other car, a woman in her 30s, (had) minor injuries, (and) has been taken to Wollongong Hospital for care.
"The driver/husband has been taken to Wollongong Hospital with chest injuries, and he left the scene well before his wife.
"It looks like the seatbelt and the airbag's gone off, and basically he's got bruising to his chest at this stage. He's in a stable condition.
"We had to stabilise his wife. She's in her 80s, she's got both her legs broken, a fractured pelvis and a nasty laceration across her throat."
