Sarah Smith, 26, is still "petrified" of the man, and his girlfriend, who attacked her at Pepe's on February 20 this year.
She is urging Wollongong police to do more so she can stop living in fear.
A NSW Police spokeswoman confirmed officers were investigating the incident but no charges had been laid.
Mrs Smith said the night out for a friend's birthday party was the first time she had gone to a club in more than two years after she had become disabled with chronic pain.
After having a couple of drinks and talking to her friends she turned around to see a man was holding her drink about 11pm.
Mrs Smith said the man's girlfriend became aggressive after she asked him politely why he had her drink.
The girlfriend said she "didn't care" if he had taken Mrs Smith's drink before she got up from the table and punched her in the face and pulled her hair.
"I was petrified," Mrs Smith said. "I couldn't defend myself. I'm very placid, I'm not a fighter."
Mrs Smith didn't retaliate, had a knee brace on and was much smaller than the woman.
The pair were quickly separated by security guards.
While still in the club, Mrs Smith started explaining what had happened to her friend when the man came up behind her and threatened to headbutt her.
"As I turned around, I saw his face come towards mine and then I was knocked out on the floor where I had a seizure," she said. "It happened so quickly.
"I was moved to a quieter location and had another seizure before paramedics arrived."
Mrs Smith was taken to Wollongong Hospital to undergo scans and to be treated for her injuries.
"I have still lost my sight in my right eye," she said. "It is like there is a piece of white cardboard in front of my eye."
Mrs Smith has to see more eye and brain specialists to assess the damage.
"My anxiety was bad before the incident but now I am constantly looking over my shoulder fearing he is there," she said.
"I'm only not very tall and I have a knee brace. I've now been diagnosed with major depressive disorder."
She said she has been living in fear the man allegedly responsible would come to her house so she put up security cameras.
"I've lost all my independence," Mrs Smith said. "I can't stay in my own home. I can't be by myself."
The police spokeswoman said the man and woman left the venue before officers arrived.
Mrs Smith said investigators had not yet arrested the man allegedly responsible for the assault despite him being known to police.
She hopes police will make an arrest soon given there were many witnesses to the assault.
The young woman also said she had been disappointed by Pepe's response in the weeks following the assault as they had not reached out to her after claiming they would.
The venue was contacted for comment.
