The incident occurred on Northcliffe Drive, near Berkeley Sports Club, about 3.35pm.
At about 5pm, the man, 21, was being loaded on to a stretcher, with paramedics conducting work on his leg.
He was transferred via road to Shellharbour Airport, and then airlifted by the Toll Helicopter to St George Hospital.
The man sustained significant leg injuries and sustained head abrasions, NSW Ambulance officials said.
NSW Ambulance Illawarra Inspector Norm Rees said a 21-year-old man was attempting to change a flat tyre on his car, when he was "struck by a second vehicle and knocked into a third".
"As a result, he's sustained multiple fractures to his left leg, a loss of consciousness, and some multiple abrasions and contusions," Insp Rees said.
"He's been stabilised at the scene... We're flying him to St George Hospital for further treatment."
The driver, an 18-year-old male, was not injured.
Police investigations into the incident are ongoing.
