This follows on from an outbreak at the Coniston centre reported on January 4.
In an update for family and carers released on Friday, three residents at the Albion Park Rail facility had tested positive since the outbreak there began on January 6.
As of Friday there were no staff that had tested positive.
The outbreak at Mt Terry, which started on Friday, has affected three residents and one staff member.
"Things are changing on a daily basis," the update stated, "and while we continue with the protocols of staff undertaking Rapid Antigen Tests every second day, and visitors having to be tested prior to visiting, we need to be prepared that one resident who tests positive, or two staff within 72 hours of working in the home will constitute an outbreak."
The update said the number of COVID cases among staff could rise as many are waiting for results of their PCR tests.
As of Friday there was one resident and one staff member with COVID at the Coniston centre.
The outbreaks mean that visits to these centres are now under restrictions.
After the Coniston outbreak, CEO Mark Sewell said strict infection safety strategies were in place.
"Warrigal has a central Clinical Compliance Team that assists every home with infection outbreaks of any kind," Mr Sewell said.
"Older people with underlying health conditions deserve protection in their homes from the devastating effects of COVID."
