Emergency services were called to the accident on the Princes Highway at Wandandian about 2.15pm on Saturday.
Initial investigations suggest a Kia Cerato sedan and a Toyota Cressida crashed head-on.
The driver of the Kia, a 58-year-old man, and the driver of the Toyota, a 17-year-old male, were both airlifted to St George Hospital.
The driver of the Kia remains in hospital in a serious but stable condition. However, the 17-year-old died on arrival.
A passenger in the Kia, a 56-year-old woman, was treated at the scene and taken by NSW Ambulance paramedics to Shoalhaven Hospital in a stable condition.
Specialist investigators from the Crash Investigation Unit are investigating the circumstances surrounding the incident.
