A superb 86th minute strike from Coniston’s Lachlan Martin propelled the District League leaders to a hugely important triumph on home turf.
The result helped Coniston finish four points clear of Bellambi, who fell to Thirroul Thunder on the final day of the season.
Coniston will now get the chance to make the jump back to the Premier League just two years after they took out the Community League title.
"I think it's great for the club," Coniston coach Rod Williams said.
"President John Karayiannis and people involved in the club had a vision and it's paid off. Its a great feeling and we will get the chance to play among the big boys now. We have a great youth grade set up so we will have something to build on."
Coniston were made to sweat for victory by a finals bound Helensburgh outfit.
Thistle led 1-0 after just two minutes when Blake Charman before Mark Picciolini leveled things up after 25 minutes. Coniston’s Dinko Terzic then had his goal cancelled out by Brad Watts in the second half.
Martin’s winner came in the dying minutes, much to the delight of the Coniston faithful.
"I felt elation," Williams said. "I don't get nervous. I have been in some big games and I tend to ponder things.
"Sometimes you get a feeling if it is going your way or not and I had a feeling we would get home."
