'Talking about Heart Health' is the theme of the week which runs April 28 to May 4.
The GPH 'Connecting Care' team in the Illawarra has partnered with the Heart Foundation to provide heart health checks, education and resources, in addition to heart health walks.
These will be conducted from the Baptist Care Centre in Warilla on Tuesday, April 30 and two days later from Shellharbour City Library.
The 'Connecting Care' and the 'Closing the Gap' teams in the Shoalhaven have also partnered with the Illawarra Shoalhaven Local Health District Heart Failure Service and Grand Pacific Health Centre Nowra, to provide heart health checks, education and resources all week.
The Closing the Gap team will also be promoting heart health checks and providing information as well as resources at the local GPH Boot Camp and Aqua Aerobics Group.

GPH chronic care facilitator Karin Robinson is encouraging local residents to get involved.
"This is a great opportunity to put your health in the spotlight and talk about your heart health. Come along for a free heart check, grab some resources or join a local walking group," Robinson said.
