Zixi Wang, 29, made a brief appearance in Wollongong Local Court on Wednesday, where she did not apply for bail and it was formally refused.
Her lawyer, Kevin Zhu, told the court Wang had been "terrified and distressed" when he had seen her in custody following her arrest on Tuesday.
"I had a short meeting with her," he said.
"She was in a very bad condition physically and mentally, she was terrified and distressed and hasn't slept for three days. I was unable to obtain instructions."
The case will return to court in Sydney in August.
Police began searching for Wang after Shuyu Zhou was found injured at the rear of a unit complex on Rose Valley Way at Zetland, in Sydney's inner east, about 8.45pm on Monday.
Police officers tried to revive the 23-year-old but she died at the scene.
Neighbours told the Sydney Morning Herald they heard "painful screaming" from the fifth-floor apartment owned by Wang in the moments before Ms Zhou was found on the concrete, next to a badly damaged Colorbond fence.
Strike Force Deepfield was established - by Redfern detectives and the State Crime Command's Homicide Squad - to investigate the "domestic-related" incident.
On Tuesday, police released images of 29-year-old Wang - also known as Jessie - who they said had an "outstanding arrest warrant in relation to the incident".
The registration of a white Mercedes was also released as part of the appeal for information.
Following a tip-off from the public, police located a white Mercedes at the Sea Cliff Bridge, north of Wollongong, about 3.30pm on Tuesday
Wang was arrested near the vehicle and taken to Wollongong police station.
She was charged with the outstanding warrant for murder, and take and drive conveyance without consent of owner.
The vehicle was seized for forensic examination.
