The 66-year-old pulled up in the driveway of an Echuca Avenue, Koonawarra, home around 6.40pm on Sunday.
Police believe that, when she stopped, she put the white Mitsubishi Pajero into neutral and then did not use the park brake.
As she got out the car began rolling backwards and she tried to get back in.
Instead, she fell to the ground and ended up trapped underneath the driver's side wheel.
Four ambulance crews rushed to the scene and found the woman trapped under the car, with the wheel on her pelvis.
She suffered pelvic and other fractures and was taken by road to the Illawarra Regional Airport.
From there the woman was airlifted to St George Hospital in a serious but stable condition.
