Police said the woman was travelling north in a car on Bolong Road at Coolangatta when it collided head on with a car travelling in the opposite direction.
The 94-year-old woman suffered head and chest injuries and died at the scene.
The male driver of the south-bound car, 41, was trapped for a short time.
He was freed and airlifted to St George Hospital.
It was the second fatal accident on Bolong Road in a month.
A woman in her 20s was also killed early on Friday morning following a crash at Yerrinbool.
Her motorbike collided with a truck on the Hume Highway near the Sierra Street overpass. She died at the scene and is yet to be formally identified.
The truck driver, a 29-year-old man, was not injured.
