Fortunately no students were crossing at the time, though the supervisor was visibly shocked after the incident which happened in front of parents and children on Tuesday afternoon.
One witness, parent Tracey Clayton, said the school crossing on Lakelands Drive was an "accident waiting to happen" with plenty of near misses in recent years as motorists failed to slow down to 40km/h - or even sped right through.
Ms Clayton was among those to campaign for a crossing supervisor at the Dapto school earlier this year, after her own brush with disaster.
"Last November my daughter and I almost got hit by a truck which didn't stop at the crossing," she said.
"We were more than halfway across, and I had to pull my daughter - who was in Kindergarten at the time - back or she would have been hit.
"Thankfully I didn't have my two younger children with me at the time or they may have been walking in front of me."
Ms Clayton rallied local MPs, and wrote to Transport for NSW, and two school crossing supervisors were appointed to the school in July this year.
"We just wanted our kids to be safe, we didn't want one of our kids to be hit before something happened," she said.
"We were grateful to get the school crossing guards, but it's terrible this has happened. Maybe we need more signage, or a zebra crossing needs to be marked out?
"We'll be looking into the options, so everyone is safe."
A Transport for NSW spokesperson confirmed it was investigating the incident on Tuesday.
"Transport for NSW is looking into the incident involving a school crossing supervisor and a motorist at Lakelands Public School on Tuesday afternoon," the spokesperson said.
"School crossing supervisors are provided to ensure the safe and efficient movement of primary and infant school children to and from school.
"They are part of a suite of measures the NSW government has implemented to keep kids safe within the state's school zones.
Other measures include 40km/h speed limits and high visibility safety features like signs, dragon's teeth road markings and flashing lights."
The school zones for Lakelands Public School operate from 8.15am to 9.15am and from 2.30pm to 3.30pm on school days.
Transport for NSW is working with the school to ensure its community is aware of the special conditions in place for motorists when school crossings are operational.
