Paramedics have vowed not to attend any jobs classed as "2-immediate" or below on June 10, in response to a lower-than-inflation proposed 1.5 per cent pay offer.
HSU Illawarra delegate Anton Jemsek said jobs in those categories may include abdomen pain, minor lacerations and minor medical conditions.
"We've had enough. We're the most undervalued and underpaid paramedics in the country," Mr Jemsek told the Mercury, adding there were too few paramedics in the Illawarra to cover the region's workload.
"We hope that it [the strike threat] doesn't go on too long and the government will realise we're serious in what we're asking for because ultimately we want to make sure our patients and people in the public aren't at a disadvantage because of the government's actions.
"Paramedics don't want to take industrial action ... this is the only way we can show the government we're serious."
Last year paramedics were awarded a 0.3 per cent pay rise. This year's proposed 1.5 per cent offer is again less than inflation, which NSW Treasury forecasts at 2.2 per cent for the coming year.
Health Services Union NSW Secretary Gerard Hayes said next week's action represented "a historic, statewide stand" by union members who had been exposed to COVID when required to transport positive patients, without adequate protective equipment.
"They were exposed to a wild and deadly disease, yet they continued to serve the people of NSW and protect their health," he said.
"To offer them a pay cut is humiliating and insulting."
NSW Health Minister Brad Hazzard said he was having discussions with the union over the planned action, but said any industrial action must follow the processes outlined by the Industrial Relations Commission.
