A taxi driver was picking up two passengers from a home in the southbound lane of the Princes Highway at about 9am.
The driver then performed a U-turn.
A truck was travelling south and hit the right side of the taxi forcing the car to spin onto the other side of the road.
The taxi driver, 77, was trapped in the car for 45 minutes before he was freed by Fire and Rescue NSW crews.
He and a male passenger were taken to hospital in stable conditions. The other driver was not seriously injured.
Rachel Capocchiano, 36, said the accident was "scary".
"I went to get into the taxi to go to Wollongong and the driver waited a while to do a U-turn," she said.
"I was looking down at my paperwork so I didn't see what happened but as we were turning a truck hit us from the side.
"The truck took our car with him and we spun a couple of times.
"[When the car was hit] I lost my breath and felt winded. It was scary. I wasn't sure if I was dead to be honest.
"I'm okay, I just have sore ribs, chest and back. I hope everyone is okay.
"I am glad my baby and children weren't in the car.
"If I was on the side of the car that was hit there would have been real damage. I am lucky."
Ms Capocchiano said her friend was in the front passenger side of the car.
"He had the full force of the driver moving onto his side of the car when the airbag deployed," she said.
Gas truck driver Steven White said the accident "happened quickly".
"There was not a lot of reaction time. There was not much I could do," Mr White said.
"It was a shock. I was a bit shaky. Everyone is okay and that is the main thing.
Mr White said paramedics assessed him but he did not have serious injuries, just some stiffness.
"I'm going to go have a nice cup of tea and a sit down," the Supagas driver said.
NSW Ambulance Inspector Matt Stirling said the taxi driver complained of rib tenderness and lower back pain before he was taken to hospital
The male passenger was also complaining of lower back pain and "tingling" in his lower body.
Lake Illawarra Police District Command Sargent Peter Northey confirmed the taxi driver performed a u-turn.
"Unfortunately, he didn't see the truck and the truck driver has driven straight into the side of the car," he said.
"The accident will be investigated by police and more than likely the offending driver will be infringed."
