The two collided t-bone style about 7am, within the Illawarra Escarpment State Conservation Area, about 1km from Bulli Pass.
The 25-year-old motorcyclist had the worst injuries, including a fractured jaw, and was given first use of the single four-wheel-drive ambulance that was able to reach the accident site.
He was later airlifted from Bulli Showground to St George Hospital.
The mountain bike rider, 42, suffered a fractured upper arm and possible a fractured wrist. He was flown to Liverpool Hospital.
Wollongong police are investigating the circumstances leading to the accident.
