The alleged victim was walking in a pedestrian laneway between Beveridge Street and Tongarra Road about 1.45pm on Saturday when he was approached by two boys, police said.
The older boy is accused of hitting the victim on the head with a metal scooter, before the boys fled.
"The injured boy was treated at the scene by NSW Ambulance paramedics, before being taken to Wollongong Hospital in a critical condition," police said.
He remained in a critical condition in hospital early on Monday morning.
Police arrested a 14-year-old boy on Saturday afternoon.
He was taken to Lake Illawarra police station, and will be dealt with under the Young Offender's Act, police said.
About 9.15pm on Sunday, police arrested a 16-year-old boy at a home in Warilla, east of Albion Park.
He was taken to Lake Illawarra police station and charged with assault with intent to cause grievous bodily harm.
He was refused police bail, and is due to appear in a children's court on Monday.
