The player was taking part in a match at Crookhaven Park on Prince Edward Ave at Culburra Beach when he reportedly tried to make a tackle.
He landed on his right side and complained of a headache, blurred vision, tingling down his left leg and rib pain.
It is understood he may have also lost consciousness after the tackle.
Ambulance officers were called and treated the boy at the scene.
He was placed in a neck collar and transported to Shoalhaven Hospital for further assessment.
