Paramedics are on scene at a serious crash north of Mittagong on the Hume Highway which left two patients trapped.
The crash happened about 1.40pm on Thursday.
A NSW Ambulance spokesman said one patient had been freed, while the other remained trapped at 2.15pm.
He said an ambulance helicopter was en route to the scene.
The highway's southbound lanes have been closed, and motorists have been urged to avoid the area.
